5 WISHES DRIVE - OFA HEALTHCARE SERVICE EVENT

WE NEED VOLUNTEERS TO STAFF 5 WISHES TABLES THROUGHOUT THE MONTEREY PENINSULA ON FRIDAY June 26th, SATURDAY June 27th, and SUNDAY June 28th!

Seventy percent of health care dollars are spent at the end of life. Often times people at end of their life are unable to communicate their wishes and must rely on loved ones to make critical medical decisions on their behalf. The 5 Wishes form, (also known as an advance healthcare directive or living will) allows people to take personal responsibility for the decisions made on their behalf and express their desires for the type of health care they wish to receive in the event that they are unable to communicate with their loved ones or doctors.

Our health care service event is a 5 Wishes Drive, similar to a voter registration drive. Visit a drive location to1) learn about and receive a free copy of the Five Wishes form, 2) share your healthcare story, and 3) learn about what you can do to support national healthcare reform.

You can participate in this event at any of the following locations on Friday, Saturday or Sunday.:
Democratic Center for Change - Monterey
Trader Joes - Pacific Grove
East Village Coffee House - Monterey
Del Monte Center (near Macy's) - Monterey
Boarders Books - Sand City
and many more locations across the Peninsula
